Meanwhile, cook garlic in hot oil in a large skillet over medium heat 1 minute or until fragrant. Stir in cream cheese, 1 cup Parmesan cheese, 1 cup pasta water, Italian seasoning, red pepper, salt, and black pepper until combined. Add spinach; cook until wilted.
Stir in orzo, stirring to combine. Add in additional pasta water, if needed, to thin the sauce to desired consistency. Sprinkle with ½ cup Parmesan cheese and crushed fried onions.
